The child physical examinations prior to attendance shall be conducted within required time periods. The physical for C2 was more than 3 months prior to attendance.

A tuberculosis (TB) screening is required every two years from the last screening. An updated TB screening for HHM 2 was completed and was overdue by 3 days. An updated TB screening for HHM 3 was completed and was overdue by 2 months.
Plan of Correction: TB test will be on time in the future.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child?s record to ensure the information is correct and shall obtain the parents signed acknowledgment of the review. The record for C4 did not have a parent?s signature for an annual review of the emergency contact information. C4 had been in care for over a year.
