Inspector Notes

An unannounced non-mandated monitoring inspection for a Conditional License was conducted on 09/30/2021. The inspector observed children during rest time and participating in free choice activities. The inspector reviewed 2 children?s records, 1 provider record, 1 assistant record and 1 pet record during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider. Standard 22VAC40-111-120-B
Based on record review and discussion, the provided failed to ensure that the assistant caregiver's record shall contain all of the elements as required by the standards.

Evidence: The record for Assistant Caregiver 1 did not contain the following: job title, date of employment, name, address and telephone number of a person to be notified in an emergency, and documentation of the education and training as required by 22VAC40-111-230.

Standard 22VAC40-111-30-A
Based on observation, record review and discussion, the provider failed to ensure compliance with the terms of the current license issued by the department.

Evidence: The current licensed issued by the department allows for the care of children aged 2 years to 12 years 11 months. The documented birthdate in the record for Child 4 verified that Child 4 was 22 months old.
Plan of Correction: Will request the license be changed to allow for younger children.
Standard 22VAC40-111-410
Based on observation and discussion, the provider failed to ensure that portable electric fans shall be securely mounted out of the reach of children and shall be equipped with a mesh guard.

Evidence: There was a portable electric fan sitting on the floor of the room where the children were napping; the fan was accessible to children.
Plan of Correction: Will remove the fan from downstairs

Based on observation and discussion, the provider failed to ensure that the telephone number for the person responsible for emergency backup care was posted.

Evidence: The telephone number for the emergency backup provider was not posted.
Plan of Correction: Will post today.
