Inspector Notes
An unannounced renewal inspection was conducted on 11/28/2022. There were 4 children, ages 17 months- 4 years, totaling 9 points and 2 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 2 caregiver records, 1 pet record and 2 medications were reviewed. The children were observed during story time, watching television, having lunch and during nap time. The inspector discussed medication and administration requirements with the provider. The inspector arrived for the inspection at 10:15 am and departed at 12:45 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
5Evidence:
1.There was a spray bottle of Kaboom cleaner in the bathroom floor that was accessible to children.
2. There were several containers of bleach, sanitizing wipes and Kaboom cleaner in the floor beside of the stove in the kitchen that were accessible to children.
3. There were 2 aerosol cans of Lysol and 2 containers of sanitizing wipes on top of the refrigerator along with the boxes of cereals.
Evidence: There was no liquid soap in the bathroom.
Evidence: The most recent 1 year rabies vaccination for Pet 1 was administered on 08/27/2021; an updated rabies vaccination was due in 08/2022.
Evidence:
1. The record for Child 2 did not contain the following information: the parent?s signed acknowledgement of the receipt of the information required by the standards; and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
2. The record for Child 4 did not contain the following information: the parent?s signed acknowledgement of the receipt of the information required by the standards (the 2nd page was missing); and documentation of the review of the child?s emergency contact information as required (the most recent review took place on 09/20/2021).
3. The record for Child 5 did not contain the following information: the parent?s signed acknowledgement of the receipt of the information required by the standards (the 2nd page was missing); and documentation of the review of the child?s emergency contact information as required (the most recent review took place on 09/13/2021).
Evidence:
1. There was a gallon zip-lock bag with numerous prescription medications for household members on a high shelf near the changing table; the medication was not locked to prevent access by children.
2. There was a bottle of Tylenol in the window sill of the kitchen; the medication was not locked to prevent access by children.
3. There were two epi-pen stored in the first aid kit located on the top of a tall filing cabinet. The first aid kit was not locked to prevent access by children.
