Inspector Notes
An unannounced renewal inspection was conducted on 11/28/2023. There were 4 children, ages 2 years- 4 years, totaling 8 points and 2 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 2 caregiver records, 1 household member record, and 1 pet record were reviewed. The children were observed in free choice play, having breakfast, participating in a learning/art project and watching educational videos. The inspector discussed staff records and the upcoming renewal application with the provider. The inspector arrived for the inspection at 8:40 am and departed at 11:00 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
5Based on record review, the provider failed to obtain a current report of tuberculosis screening every two years from the date of the first screening for each caregiver and household member.
Evidence:
1. The most recent tuberculosis screening in the record for Caregiver 2 was dated 05/12/2021; there was not a follow-up screening obtained by 05/12/2023 available for review during the inspection.
2. The most recent tuberculosis screening for Household Member 1 was dated 04/30/2021; there was not a follow-up screening obtained by 04/30/2023 available for review during the inspection.
Evidence: The record for Child 5 contained a documented first day of attendance of 05/01/2023; there was not a physical examination in the record for Child 5.
Evidence: The bathroom did not contain liquid soap.
Based on record review, the provider failed to ensure that each child?s record shall contain all of the elements as required by the standards.
Evidence:
1. The record for Child 1 did not contain the first date of attendance.
2. The record for Child 2 did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
3. The record for Child 3 did not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached (there was only one designated person listed).
Evidence: The most recent immunization record available for Child 2 was dated 10/22/2022. Documentation of additional immunizations were not obtained every six months for a child under the age of 2 years.
