Inspector Notes
An unannounced on-site monitoring inspection was conducted on 06/13/2025. The on-site inspection began at 9:05 am and ended at 10:00 am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver; the point total was 7. The inspector reviewed 5 children?s records, and 1 caregiver record. The inspection included document review, tour of the home, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law and are documented on the violation notice issued to the program.
Violations
3The provider shall obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance.
There was not a physical exam record available for Child 3 who has been in care for 2 years.
There was no liquid soap in the bathroom used by children.
Children?s records are required to contain the name, address and telephone number of two designated persons to contact in case of an emergency.
The record for Child 2, who has been enrolled for 15 months, did not include two designated persons to contact in case of an emergency.
