Inspector Notes

An unannounced renewal inspection was conducted on 12/16/2022 from 10:30 am - 11:35 am. At the time of entrance there were five children in care with the provider. The ages of the children counted for a total of 14 points. The inspector observed free play, one diaper change, infant nap and the provider holding and feeding an infant.
Records were reviewed for three children, the provider and one household member.

Information gathered during the inspection determined non-compliance with applicable standards and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-800-690-D
Based on observation, the provider failed to ensure that the diapering surface shall be nonabsorbent and washable.

Evidence:
1. The pad on the diaper changing table has two tears resulting in the foam padding being exposed.
a. The diaper pad can not be properly cleaned and sainted when the padding is exposed.
Plan of Correction: A new diaper pad will be obtained.
Standard 8VAC20-800-960-E
Based on observation, the provider failed to ensure that bottles shall be labeled with the child's full name and the date, if more than one infant is in care.

Evidence:
1. Two bottles were not dated and labeled with the child's name.
a. The provider stated there were two infants in care who were fed from bottles of formula.
Plan of Correction: All bottles will be labeled and dated, going forward, if more than one infant is in care.
