Inspector Notes

An unannounced , on-site renewal inspection was initiated on 11/18/2024 and completed on 11/18/2024. The on-site inspection began at 9:08 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 caregiver. The point total was 10. The inspector reviewed 5 children's records on-site and 2 caregiver/household member records on-site and electronically on 11/18/2024.

Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in 8VAC20-800-60-B 2 to ensure the information is correct , and obtain the parent's signed acknowledgment of the review.

Written documentation of an annual review and update of emergency contact information was not available for child 1, child 2 and child 3. All of these children have been in care more than one year.
Plan of Correction: The provider stated the each child's record will be reviewed by the parent and updated if needed. Each parent will sign the record to acknowledge review.
