During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 03/27/2025 and completed on 03/27/2025. The on-site inspection began at 10:30 am and ended at 11:40 am. The inspector reviewed compliance in the areas listed above. There were 6 children present with 1 caregiver; the point total was 12. The inspector reviewed 6 children's records and 1 caregiver record on-site.

Standard 22.1-289.057-A
Each child day program that is licensed pursuant to this chapter and any program described in subdivision A 4, B 1, or B 5 of ? 22.1-289.030 that serves preschool-age children shall develop and implement a plan to test potable water from sources identified by the U.S. Environmental Protection Agency as high priority, including bubbler-style and cooler-style drinking fountains, kitchen taps, classroom combination sinks and drinking fountains, home economics room sinks, teacher's lounge sinks, nurse's office sinks, classroom sinks in special education classrooms, and sinks known to be or visibly used for consumption.

The program is required to submit results of portable water testing to VDH. The program did not have evidence of submiting test results to VDH.
Plan of Correction: The provider stated that she would reach out to a certified lab to inquire about testing of potable water used by the FDH.
Standard 8VAC20-800-210-A
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.

The provider does not have written documentation of completion of 16 hours of annual training for 2024.
Plan of Correction: The provider stated she can not locate her training certificates for 2024.
The provider stated she has begun taking training classes for 2025 and will file certificates in her record as received.
