Inspector Notes

An unannounced monitoring inspection was conducted on 11/28/2022 from 9:40 am - 12:10 pm. At the time of entrance there were 22 infants & toddlers in care with 6 staff.
Children were observed during outdoor play, free play, lunch and nap.
Records were reviewed for four children and three staff.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center failed to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
1. Staff 1, hire date 07/12/2022, lacks the results of a central registry check from another state she has resided in within the past five years.
2. Administrative staff confirmed that a request for an out-of-state central registry check had not been made for staff 1.
Plan of Correction: An out-of-state central registry check was requested immediately. Results received as "clear" on 12/07/2022.
Standard 8VAC20-780-440-J
Based on observation, the center failed to ensure there shall be at least thirty inches of space between the service side of an occupied crib and any other furniture where that space is the walkway for staff to gain access to any occupied crib.

Evidence:
1. The service side of one occupied crib lacked the required 30 inches of space between cribs.
a. The service side of the crib was approximately 12 inches from another crib.
Plan of Correction: All staff will be reminded to position occupied cribs to ensure at least 30 inches of space on the service side.
