Inspector Notes

An unannounced renewal inspection was conducted on 12/19/2023 from 11:00 am - 1:35 pm. At the time of entrance there were 19 infants and toddlers present with 7 staff.
Children were observed during late morning program time, lunch and nap. Records were reviewed for 5 children and 5 staff.

Based on the information found violations were observed in the areas of administration and nap time ratio. These violations are listed on the violation notice issued to the facility and were reviewed with the program director at the conclusion of the inspection.

Standard 8VAC20-780-350-I
Based on observation and interview, the center failed to ensure that for children ages 16 months through preschool age, during the designated rest period, when children are resting or in an inactive state, the following staff-to-child ratio shall be maintained: one staff per 10 children.

Evidence:
1. When the inspector reviewed the toddler classroom at approximately 1:05 pm, 13 toddlers were observed napping with 1 staff present in the classroom. (staff 1)
a. Staff 1 stated that she did not know where the 2nd staff was (staff 2) and did not know how long she had been out of the classroom.
2. Staff 2, who was assigned to the toddler classroom during nap, was observed in the adjacent infant classroom at approximately 1:05 pm.
a. Staff 2 was sitting in a rocking chair holding an infant.
b. Staff 2 acknowledged that she had left the toddler classroom and was assigned to the toddler classroom during nap.
c. Administrative staff stated that staff 2 was supposed to be in the toddler classroom during nap with staff 1.
Plan of Correction: On the day of the inspection there were sufficient staff to meet all required staff-to-child ratios in both classrooms. Administrative staff has spoken with all staff to clarify nap time ratios and the requirement that staff remain in their assigned classrooms at all times.

Standard 8VAC20-780-40-M
Based on review and interview, the center failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

Evidence:
1. The written allergy list was not maintained in the infant classroom.
a. Classroom staff and the program director stated that the allergy list had not been placed in this classroom.
2. The written allergy list in the toddler classroom was not dated as required.
Plan of Correction: The allergy list will be dated and placed in both classrooms.
The allergy list had not been placed in the infant classroom as none of the children in this group have any allergies, sensitivities or dietary restrictions.
