Inspector Notes

A renewal inspection was initiated and concluded on March 22, 2022. There were 62 children present with ten staff. Children present ranged in age from 2.5 years through six years old. A sampling of children and staff records were reviewed. A walking tour of the program inside and outside was completed. First aid kits and emergency supplies were reviewed on both floors of the center. A single violation regarding resilient surface was observed and reviewed with the program director during the exit interview.

Based on observation and interview the center failed to ensure that resilient surface is maintained as required.
Evidence:
1 - The poured-in-place resilient surface has begun to separate creating cracks throughout the outside play areas. The cracks vary in width from approximately 1/4 inch up to approximately 4 inches wide.
a. The cracks create a tripping hazard and
b. negate the resilient surface effectiveness in the areas where they are present.
2 - The program director confirmed that the resilient surface has separated in areas throughout the outside playspace.
Plan of Correction: Center management will contact the resilient surface manufacturer to make arrangements for the resilient surface to be repaired.
