During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on completed on 6/5/2025, as a part of licensure period. The on-site inspection began at 10:14 and ended at 12:45. The inspector reviewed compliance in the areas listed above. There were 129 children present and 18 staff. The inspector reviewed 9 children?s records and 6 staff records electronically on June 6, 2025. This inspection included documentation review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five days from today, which will be the close of business on 6/28/2025. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in safe condition. There is a safety gate blocking the direct egress to outside between the two?s and the toddler care spaces. There is one loose fitting crib sheet in the infant room, posing an entanglement hazard.
Plan of Correction: The crib sheet was immediately replaced with a properly fitted sheet, and all infant staff were reminded of appropriate crib safety standards. A weekly crib inspection protocol is now in place. Regarding the safety gate, we were unaware that this was a violation at the time of inspection, and we are currently awaiting clarification from the Fire Marshal to determine whether a baby gate may be used to safely and compliantly separate the two classrooms. We will take any corrective action necessary based on the Fire Marshal?s guidance.
Standard 8VAC20-780-330-B
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. There is less than the required resilient surface under around and into the fall zone of climb and slide equipment on the outside play space. Additionally, children were observed climbing on playground equipment that did not have any resilient surface.
Plan of Correction: We have placed an order for additional mulch to meet the required resilient surfacing standards under and around all climbing and slide equipment. The delivery and installation are scheduled to be completed no later than the end of July 2025. Children will not be permitted to use the affected equipment until all required surfacing is in place.
Standard 8VAC20-780-450-A
During the designated rest period children shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. In the toddler room there were 13 children napping without a top or bottom cover.
Plan of Correction: As discussed with the licensing inspector at the time of the visit, all children are regularly provided with both a top and bottom cover. On the day of the inspection, due to a recent lice outbreak, linens had been removed and were in the dryer as part of precautionary sanitation measures. The situation was rectified immediately and moving forward, we will ensure back-up linens are readily available in each classroom to prevent any disruption in compliance under similar circumstances.
