Inspector Notes

A renewal inspection was conducted on March 8, 2022 and concluded on March 8, 2022. On March 8, 2022 the licensing inspector was onsite from approximately 2:20 pm until 4:45 pm.The director assisted with the inspection. There were 19 children present, ranging in ages from four months to five years, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
Description: Based on a review of seven staff records, the center did not ensure that five staff member had a central registry finding within 30 days of employment.

Evidence:

1. The record for staff #1, hired on 09/13/21, contained the results of a central registry finding dated 12/15/2021, exceeding 30 days from hire.
2. The record for staff #2, hired 10/12/2021, contained the results of a central registry finding dated 12/07/2021, exceeding 30 days from hire.
3. The record for staff #3, hired 01/13/2022, did not contain documentation of a central registry finding, exceeding 30 days from hire. The center did not have any further documentation of contact to follow up on the status of the finding.
4. The record of staff #5, hired 01/25/2022, did not contain documentation of a central registry finding, exceeding 30 days from hire. The center did not have any further documentation of contact to follow up on the status of the finding.
5. The record of staff #6, hired 02/17/2022, did not contain documentation of a central registry finding, exceeding 30 days from hire. The center did not have any further documentation of contact to follow up on the status of the finding.

Plan of Correction: Per the administration: I will follow up on the status of the missing central registry checks. In the future I will document when the central registry checks are requested and follow up at 30 days if they are not back.

Based on review of four children's records, the center did not obtain documentation of immunizations for two children's records before the children attended the center.

Evidence:

1. The record for child #3, enrolled 02/07/2022, did not contain documentation of immunizations.
2. The record for child #4, enrolled 02/07/2022, did not contain documentation of immunizations.
Plan of Correction: Per the administration: I will ask the parents to provide the immunization record for the files.

Based on review of four children's records, the center did not obtain documentation of a physical for two children's records before the child's attendance or within 30 days after the first day of attendance.

Evidence:

1. The record of child #3, enrolled 02/07/2022, did not have documentation of a physical exceeding 30 days from the first day of attendance.
2. The record of child #4, enrolled 02/07/2022, did not have documentation of a physical exceeding 30 days from the first day of attendance.
Plan of Correction: Per the administration: I will ask the parents to provide the physicals for each child.
Standard 8VAC20-780-70
Based on review of seven staff records, the center did not obtain all required documentation for one staff record.

Evidence:

The record for staff #2, hired 10/12/2021, did not contain documentation of a tuberculosis screening. Staff hired prior to 10/13/2021 were required to obtain a tuberculosis screening within 21 days of hire.
Plan of Correction: Per the administration: I will have the staff provide documentation of their tuberculosis screening for the record.
