Inspector Notes

An unannounced complaint inspection was conducted today, February 29, 2024, in response to allegations regarding behavioral guidance, ratio, cleanliness, and forbidden actions received by the department on February 26, 2024. The director was present during the inspection. The inspector was on site from approximately 1:20 pm until 2:00 pm. Interviews with staff were conducted and observations were made. One staff record was reviewed and two children?s records. The preponderance of evidence gathered during the investigation does not support the allegations of non-compliance with standards or law and therefore was not valid.

Based on review of two children?s records, the center did not obtain documentation of a physical examination before the child?s attendance or within 30 days after the first day of attendance.

Evidence:

1. The record of child #1, enrolled 02/23/2022, did not contain documentation of a physical examination, exceeding 30 days from the date of enrollment.
2. The director stated she did not have documentation of a physical for child #1.
