Inspector Notes

An unannounced complaint inspection was conducted today, April 2, 2024, in response to allegations regarding forbidden actions received by the department on March 29, 2024. The director was present during the inspection. The inspector was on site from approximately 3:20 pm until 4:15 pm. Interviews with staff were conducted and observations were made. The preponderance of evidence gathered during the investigation does not support the allegations of non-compliance with standards or law and therefore was not valid. Violations not related to the complaint, but identified during the investigation can be found on the violation notice.

Standard 8VAC20-780-350-B-3
Based on observation and interview, the center did not maintain the required staff to children ratio for children 2 years old.

Evidence:

1. The required staff-to-children ratio for children 2 years of age is one staff for every eight children. At approximately 3:30 pm on April 2, 2024, 14 children were observed in care with staff #3 supervising.
2. Staff #3 stated all the children were 2 years in age. One additional staff was required.
3. The director acknowledged the two?s classroom was out of ratio and confirmed no additional staff was available.
Plan of Correction: Per the director: We are in the process of hiring additional staff. One of our staff had to leave early today for a doctor's appointment.

Evidence:

1. The required staff-to-children ratio for children 3 years of age to school age eligibility is one staff for every ten children. At approximately 3:30 pm on April 2, 2024, 11 children were observed in care with staff #4 supervising.
2. Staff #4 stated all the children were four years in age. One additional staff was required.
3. The director acknowledged the four?s classroom was out of ratio and confirmed no additional
staff was available
