During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on March 23, 2026, and completed on March 23, 2026, as a part of the licensure period. The on-site inspection began at 1:00 pm and ended at 2:40 pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and six staff. The inspector reviewed five children?s records and five staff records on-site.
This inspection included document review, tour of the facility, observations, and interviews.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record of child #4 contained documentation of immunizations that were dated two months and 26 days after the first day of attendance.
Plan of Correction: Per the director: We have been reviewing our files to ensure they are up to date. we will make sure immunizations are collected by the first day of attendance.
Standard 8VAC20-780-260-A
The center shall provide the licensing representative with an annual fire inspection report from the appropriate fire official having jurisdiction. The fire inspection report exceeded the yearly requirement by 19 days.
Plan of Correction: Per the director: I will contact the Fire Marshall and schedule them to come out.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Child #3 was observed asleep and unbuckled in the DuoGlider-swing. The equipment?s manufactures tag states: ?Warning, this product is not safe for sleep. To prevent serious injury or death from infants falling or being strangled in straps: always use restraints and adjust to fit snugly at all times?.
Plan of Correction: The child was removed from the swing and placed in the crib. Per the director: we will remove the piece of equipment from the facility.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days, or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The medication authorization for child #6 expired two months and 24 days ago. The medication had not been returned to the parent and was on site.
Plan of Correction: Per the director: we will contact the parent and obtain the renewed authorization for the parent.
