Inspector Notes

An unannounced monitoring inspection was conducted on May 23, 2023, from 8:30 am until 11:45 am. There was 1 child present (4 points) with 1 caregiver supervising. The home, children's records, caregiver records, household member records, evacuation logs, attendance logs, cots and cribs were reviewed. There were no medications on site. Based on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:

Based on a review of household member records, the provider failed to obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
