During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/11/2025. The on-site inspection began at 9:38am and ended at 11:01am. The inspector reviewed compliance in the areas listed above. There were 8 children present and one caregiver, the point total was 17. A 2nd caregiver arrived at 10:20am. The inspector reviewed 7 children?s records and 2 caregiver/household members records on-site on 2/11/2025. This inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/18/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-800-210-A
Repeat Violation
Caregivers shall obtain a minimum of 16 clock hours of training annually.

Caregiver #1 did not obtain any training hours in 2024.
Caregiver #2 obtained only 5 hours of training in 2024.
Plan of Correction: All caregivers will obtain a minimum of 16hrs. annually.

Poisonous substances and materials shall be stored in an area inaccessible to children.

Poisonous cleaning products were in the basement bathroom that children use behind a shelf on the tub and within reach of children.
Plan of Correction: Cleaning wipes and tile cleanerwas moved from out of sight
to out of reach.

Repeat Violation
The provider shall ensure that a caregiver does not exceed 16 points worth of children when
supervising children.

From approximately 9:38am until approximately 10:20am, Caregiver #2 supervised 8 children alone totaling 17 points, which exceeded the point system by 1 point.
Plan of Correction: Household member will remain with his child, in the event assistant
is running late.

Repeat Violation
Children's records are required to contain all required information.

The records for Child #5 and Child 7 were missing one parent's place of employment address and phone number, office address of the children's physician and name and policy number of the children's medical insurance. The children were present during the inspection and had been in care for over four months.
