During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/26/2026. The on-site inspection began at 9:17am and ended at 10:40am. The inspector reviewed compliance in the areas listed above. There were 6 children present and two caregivers; the point total was 16. The inspector reviewed 3 children?s records and 2 caregiver/household members records on-site on 3/26/2026.
This inspection included document review, tour of the home and observations

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/02/2026. A POC submitted after this date will not appear on the public website.

The record for Child #3 was missing one emergency contact address and telephone number and a second emergency contact name, address and telephone number. The child was present during the inspection and had been in care for approximately four months and three weeks.

Diaper products do not need to be kept locked, but they must be inaccessible to children.

One diaper product was accessible to children in a lower level drawer on the tub in the bathroom children use.
Plan of Correction: Diaper cream was removed.

Shelter-in-place procedures shall be practiced a minimum of twice per year.

The provider practiced the shelter in place procedures once in 2025.
Plan of Correction: Document will be edited to reflect Shelter-in-place and Fire drills.

Documentation is required to be maintained of emergency evacuation drills.

The provider did not document the evacuation drills in August 2025 or January 2026. She stated she did them the same day she did other drills.

Before attendance, documentation of immunizations that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations is to be obtained for each child.

The documentation for Child #3's immunizations was received 3 months and 3 weeks after attending the family day home.
