Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 12:00pm-2pm. There were 3 enrolled children and 1 resident child (8 points) directly supervised by the provider. The home, 1 caregiver record, 3 children?s records, 1 household member?s record, and evacuation drills were inspected. Children were observed playing freely and preparing for naptime. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 caregiver does not have current certification in cardiopulmonary resuscitation (CPR) issued within the past two years.

Evidence:
1. The CPR certification in the record for Caregiver #1 expired on 11/26/2023.
Plan of Correction: The provider will register for class and complete the certification by the end of this month.
Standard 8VAC20-800-180-A
Based on record review, the provider did not obtain a current report of tuberculosis screening every two years from the date of the first screening for 1 caregiver and 1 household member.

Evidence:
1. The most recent tuberculosis screening in the records for Caregiver #1 and Household Member #1 were dated 5/25/2021.
Plan of Correction: Current tuberculosis screenings will be completed over the weekend.

Based on record review, the provider did not review with the parent the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgement of the review for 3 of 3 children.

Evidence:
1. The most recent review of emergency information for Child #1, 2, and 3 was dated 12/09/2022.
Plan of Correction: The provider will have parents review the emergency information and sign the form today.
