Inspector Notes
A renewal inspection was conducted Thursday, January 5, 2023. The inspection was initiated at approximately 10:15am and concluded at approximately 12:00 PM. There four non-resident children and one resident child present, ranging in ages from three years to five years, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of four children's records and one caregiver?s record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

1. The record for Child #1 (first date of attendance 07/12/2022) did not contain the physical address for one of the two emergency contacts listed.
2. The record for Child #4 (first date of attendance 09/22/2022) did not contain one of the required two emergency contacts? names, address and phone number.
Evidence:
1. The documentation of drills completed in 2022 did not document an evacuation drill for December 2022.
2. The caregiver stated an evacuation drill was conducted in December 2022 but was not documented.
