Inspector Notes

A renewal inspection was initiated on 02/23/2022 and concluded on 03/08/2022. The facility submitted documentation to the inspector on 02/23/2022 and the inspector conducted an unannounced inspection on-site on 02/28/2022 from approximately 3:45pm to 4:45pm. On 02/28/2022, there were 18 children present with three staff directly supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
Based on a review of three staff records and interview, the center did not ensure one staff member had a central registry finding within 30 days of employment.

Evidence: 1) The central registry finding in the record for Staff #1, hired on 10/12/21, was dated 11/30/21. 2) During interview, a member of management confirmed the central registry finding for Staff #1 was not received within 30 days of employment. The center did not follow up with the Office of Background Investigation (OBI) to inquire about the results until 11/24/21.
Plan of Correction: In the future, if the results are not received within 30 days of employment, the center will follow up with OBI in a timely manner.
Standard 8VAC20-780-530-C
Based on a review of records, interviews, and observations, the center did not ensure that there should be at least two staff members who have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care and current certification in first aid.

Evidence: 1) On 02/28/22, Staff #1, Staff #2, and Staff #3 were observed at the center during the inspection. 2) The record for Staff #1, hired on 10/12/21, and the record for Staff #3, hired on 09/23/21, did not contain documentation that either staff member have current certification in CPR or first aid. 3) During interview, a member of management confirmed Staff #1 and Staff #3 do not have current certification in CPR or first aid. There shall be at least two staff members who meet the requirements of subsection A of this section present on the premises during the center's hours of operation, on fieldtrips, and wherever children are in care.
Plan of Correction: All staff members will be required to obtain CPR and first aid certification moving forward.
