Inspector Notes

A monitoring inspection was conducted on May 14, 2024, as a follow-up to the issuance of a provisional license. The inspector was on site from approximately 4:00 pm until 5:00 pm. The site director
was present during the inspection. There were 21 children present, ranging in ages from six years to twelve years, with two staff supervising. One group of children were observed playing on Ipads, working on homework and coloring while a second group participated in a soccer activity.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records were reviewed. Two staff records were reviewed remotely on May 17, 2024.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of two staff records, the center did not ensure one staff obtained and submitted the results of a follow-up tuberculosis screening at least every two years from the date of the initial screening.

Evidence:

The record of staff #2, employed 10/28/2021, contained documentation of a tuberculosis screening dated 01/20/2022, exceeding two years
Plan of Correction: Per the administrator: Staff #2 will obtain a current tuberculosis screening.
