Inspector Notes

A Subsidy Health and Safety monitoring inspection was conducted on 07/18/2022 to review health and safety requirements. This inspection took place between the hours of approximately 10:45 a.m. and 11:20 a.m. There was 1 caregiver with 3 children for a total of 8 points within supervision guidelines. Children creating a craft and napping. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant was inspected. There were 0 medications on site at this time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview, the vendor did not obtain all children?s information for each enrolled child.

Evidence:
1. The vendor stated that she did not obtain proof of identity for Child #1.
2. The vendor stated that she did not obtain proof of identity for Child #2.
3. The vendor stated that she did not obtain proof of identity for Child #3.
4. The vendor stated that she did not obtain proof of identity for Child #4.
Plan of Correction: I will ask the parents to bring in the birth certificates.
Standard 8VAC20-790-360-I
Based on observation, stuffed toys were used by children less than two years of age while sleeping.

Evidence: Child #2 (age: 3 months) had a stuffed toy in their crib while sleeping.
Plan of Correction: Corrected during inspection. I will not put toys in the crib anymore.
