Inspector Notes

This monitoring inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in person inspection was conducted of the program.
The inspection was initiated on September 27, 2022 and concluded September 27, 2022. There were 11 children present, ranging in ages from 6 months to age 4, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, personnel, physical health of caregivers, caregiver training, physical equipment/ environment, care of children, preventing spread of disease, emergencies and nutrition. A total of 5 child records, I provider and 1 assistant record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-680-A
Based on observation and provider interviews, the provider did not ensue to wash their hands with liquid soap and warm running water as required.

Evidence: The provider was observed conducting a diaper change for child #1. Immediately after the diaper change, the provider placed child #1 in a bouncy seat and then used hand sanitizer on their hands. The provider did not wash their hands with liquid soap and warm running water as required after helping a child with diapering.
Plan of Correction: The provider will be sure to wash hands and hers, even though the provider had on gloves.
Standard 8VAC20-800-680-B
Based on observation and provider interviews, the provider did not ensue children?s hands are washed with liquid soap and warm running water as required.

Evidence: Child #1 was observed during a diaper change. Immediately after the diaper change child #1 was placed in a bouncy seat and the provider did not wash the child?s hands with liquid soap and warm running water as required after diapering or toileting.
Plan of Correction: Provider will be sure to wash infant hands.
Standard 8VAC20-800-690-D
Based on observation and provider interviews, the provider did not ensue the diapering surface shall be separate from the kitchen, food preparation areas, or surfaces used for children?s activities; nonabsorbent and washable; and cleaned and sanitized after each use.

Evidence: The provider was observed during a diaper change for child #1. The provider placed child #1 on an absorbent blue and white pad, that had been placed on the carpeted floor which is an area used for the infants and toddlers activities.
Plan of Correction: Provider will not change in activity area.

Based on documentation review and provider interviews, the provider did not ensue emergency response drills included all required documentation.

Evidence: The evacuation drills from March 2022 to August 2022 does not include the following information;
1.Identity of the person conducting the drill;
2.The method used for notification of the drill;
3.The time it took to complete the drill;
4.Problems encountered, if any
5.Weather conditions.

Standard 8VAC20-800-960-E
Based on observation and provider interviews, the provider did not ensue bottles shall be refrigerated and labeled with the child?s full name and the date, if more than one infant is in care.

Evidence: There were two bottles observed on a filing cabinet in the infant/ toddler area without the infant?s names and dates. Provider confirmed they belonged to two different infants.
Plan of Correction: The bottles were labeled the same day.
