An unannounced complaint inspection was initiated on and completed on November 16, 2023, regarding concerns related to Administration and Care of Children.
During the inspection the following were completed interviews with providers, review of provider documentation and observations of children. The areas of Administration and Care of Children were reviewed as a part of the inspection There was a preponderance of evidence related to Administration and therefore, the complaint is determined to be valid.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Based on interviews and documentation review, the provider did not ensure compliance with these the terms of the current license issued by the department.

Based on interviews and documentation review, the provider did not ensure the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.
Evidence: The licensed capacity is 12 and on November 14, 2023 there were 14 children from approximately 4:00 PM to 4:16 PM and on November 15, 2023 there were 13 children from 4:15PM util 4:30 PM.
