Inspector Notes

A monitoring inspection was initiated on 9/8/2023 and concluded on 9/8/2023. There were 32 children present with eight staff supervising and one administration staff available. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. The inspection started at 11:35 am and concluded at 2:35 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

(REPEAT VIOLATION)
Based on record reviews the center failed to ensure that any employee of a licensed, child day program, must be denied continued employment if, the licensed child day program does not have a central registry finding within 30 days of employment.
Evidence:
Staff #3 (hire date 8/17/23) and staff #5 (hire date 3/22/23) do not have central registry checks on file. Both staff were working on the date of the inspection.

The center failed to ensure that the center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

Based on observation the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
The side of the building next to the door used by the infants/toddlers to go outside has siding that is cracked and holes. These areas are sharp and could snag clothing and skin.

Based on observation the center failed to ensure that each restroom area provided for children shall have sinks located near the toilets and that are supplied with running warm water that does not exceed 120 F.
Evidence:
The bathroom used by the preschool and two year old class has a water temperature measuring 123 degrees F.
