Inspector Notes
A renewal inspection was initiated on 3/6/24 and concluded on 3/6/24. There were 32 children present with 10 staff supervising and two office staff present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed. The inspection started at 9:20 am and concluded at 12:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Standard 22.1-289.011-F
Based on observation the center failed to ensure that all documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
Evidence:
The most current violation notice dated 9/8/23 was not posted in a conspicuous place at the center.
Plan of Correction: Corrected during inspection. Will ensure all required documents are posted.
Standard 22.1-289.023-A
Based on observation the center failed to ensure that the notice of the Superintendent's intent to take any of the actions enumerated in subdivisions B 1 through 6 shall be provided by the Department, and a copy of such notice shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.

The Notice of Intent was not posted in a prominent place at each public entrance.

Standard 8VAC20-780-500-B
Based on observation the center failed to ensure that the diapering surface was nonabsorbent.
Evidence:
The diapering pad used in the infant area has tears causing the surface to be absorbent.
Plan of Correction: Will purchase a new changing pad.

Based on observation the center failed to ensure that A copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
Evidence:
The special order dated 8/15/23 was not posted at the center at each public entrance.
