During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 3/6/25. The on-site inspection began at 9:30 am and ended at 12:45 pm. The inspector reviewed compliance in the areas listed above. There were 24 children present and 11 staff. The inspector reviewed five children?s records and six staff records on-site and electronically on 3/6 and 3/7/25. This inspection included: document review, tour of the facility,and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-160-A
Each staff shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming in contact with children.
The screening for staff #2 was completed 11 days after hire.
Plan of Correction: Staff was able to obtained a TB screen on 3/6/25. Director will ensure that all new staff have a TB screen prior to comin gin contact with children.
Standard 8VAC20-780-210-A-4-b
Within six months of being promoted, a minimum of 24 hours of training shall be received related to the care of children. Such training may take place on site while not supervising children.
Staff #4, who has been actively working at the center for the past 13 months, did not have a minimum of 24 hours of training documented prior to being promoted.
Plan of Correction: The staff is enrolled in college courses, 24 hours of training will be documented in the staff's file.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.
The most current fire inspection available for review was dated 10/4/2023.
Plan of Correction: A fire marshal report/inspection was conducted on 3/7/25.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

Child #4, who has been in care 19 months, had a medication authorization that expired three months ago.
Plan of Correction: A renewed medication authorization was received by the parents and physician on 3/7/25.
