Inspector Notes

A renewal inspection was initiated on 01/22/2024 and concluded on 01/22/2024. There were 68 children present, ranging in ages from5 years to 11 years , with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 8 staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on staff record review, the center failed to obtain results of a follow-up tuberculosis screening at least every two years from the date of the first initial screening.

Evidence:
1. The record of staff #2 did not contain an updated tuberculosis screening or test. The date of the tuberculosis screening in the file was dated 11/15/2021.
Plan of Correction: The staff will be sure to have the staff get their updated screenings/tests before the two year expiration.
