An unannounced, on-site monitoring inspection was initiated on 7/24/2024 and completed on 7/24/2024. The on-site inspection began at 10:00amand ended at 10:40am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregiver;
the point total was 11. The inspector reviewed 3 children?s records, 1 staff record , and 1 household member record on-site on 7/24/2024. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

1. Child #3 (doe: 6/3/2024) did not have documentation of the Medication Authorization, Emergency Preparedness plan, and the acknowledgement of Liability Insurance form on file at the time of the inspection..
