Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including phone contacts, and documents review.
A monitoring inspection was initiated on 11/08/2021 and concluded on 11/15/2021. The director was contacted by phone and a virtual inspection was conducted. There were 13 school-age children present, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and three staff records were reviewed. New requirements became effective on 10/13/21. The facility has not fully complied with the following requirement; 8VAC20-780-240: The following needs to be included in orientation prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities: Prevention of sudden infant death syndrome and use of safe sleep practices; Prevention of shaken baby syndrome and abusive head trauma, including procedure to cope with crying babies or distraught children. Prevention of and response to emergencies due to food and other allergic reactions including: Recognizing the symptoms of an allergic reaction; Responding to allergic reaction; Preventing exposure to the specific food and other substances to which the child is allergic; and D. Preventing cross contamination; The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.
The inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence:
1. The inspector reviewed an allergy plan for child #1 for whom an epi-pen had been prescribed. 2. The inspector reviewed staff #2's red cross epi-pen training certificate; MAT training had not been received.
3. The director confirmed that no staff person at the site had the required MAT training.
