During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 6/9/2025. The on-site inspection began at 8:45 am and ended at 10:55 am. The inspector reviewed compliance in the areas listed above. There were 14 children present and two staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included: document review,
tour of the facility, interviews,
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/17/25. A POC submitted after this date will not appear on the public website.
Violations
4The findings from the most recent inspection, dated 6/7/24, were not posted in a conspicuous place on the premises.
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child # 1, who had been enrolled for four years and child #6 who had been enrolled for two years.
Child #1, who has been ion care for five years and child #2, who has been in care for one week, have emergency medications at the center. There are no MAT trained staff at the center.
560.F.5-When centers choose to provide meals or snacks, a menu listing foods to be served for current one week period shall be dated, posted, list any substituted food and kept on file for one week.
The center did not have a menu posted.
560.F.1-Centers shall follow the most recent, age appropriate nutritional requirements of the USDA. Snacks shall consist of two of the five food components.
The snacks consisted of crackers and water.
