Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in-person tour of the program.

An initial Subsidy monitoring inspection was initiated on 12/21/2021 and concluded on 12/27/2021. The provider was contacted by telephone and a virtual inspection of documentation was conducted. On 12/27/2021 the inspector completed the "in-person" component of the inspection at the provider's home. There were no children present and are no children currently enrolled in the program. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. One provider record and one household member record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the vendor does not have a battery-operated radio available for use in an emergency or if power is lost.
Evidence:
The home was inspected on 12/27/2021.
When the inspector asked to see the battery-operated emergency radio, the vendor stated that she did not have one.
Plan of Correction: The vendor stated that she would purchase a battery operated radio to be used for emergencies by the end of the week. A picture and receipt will be forwarded to the inspector once it is purchased.
