Inspector Notes

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the monitoring) inspection. Subsidy standards that are not covered in voluntary registration standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-180-B
The Vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. During the inspection 2 children were observed in care and there was not an attendance sheet with their name that marked their arrival time.

Standard 8VAC20-790-190-A
Vendors shall maintain, and keep at the family day home, written or electronic information for each enrolled child. The following was missing from the childrens records
1.Child #1 `s record did not include proof of the child?s identity.
2.Child #1?s and #2?s records did not include the following
a .written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection for the provision of medical treatment on religious or other grounds;
b. written authorizations that the vendor will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the vendor

Standard 8VAC20-790-190-B
The child?s information shall include proof of the child?s identity. The record for Child #1 did not include documentation of proof of identity.

Standard 8VAC20-790-360-I
Pillows and filled comforters shall not be used by children less than two years of age while sleeping or resting, including quilts, sheepskins, or stuffed toys. Child #2, less than age 2 was observed sleeping in a crib with a stuffed toy.

Standard 8VAC20-790-390-E
The diapering surface shall be separate from surfaces used for children?s activities, shall be cleaned and sanitized after each use and be nonabsorbent and washable. Vendor noted they change diapers by placing the diaper pad on the sofa in children?s activity area and after diapering they use a disinfectant spay to wipe it down but do not clean it with soap water as required. The diaper pad was observed and there was a rip and therefore it was no longer nonabsorbent.

The emergency response drills shall be practiced at least monthly. The Vendor did not have documentation of practicing evacuation drills in March, April and May 2025.
