Inspector Notes

An unannounced monitoring inspection was initiated and concluded on 10/24/2022. There were 14 children present with 3 staff supervising. The physical plant, children's records, staff records, evacuation drills, first aid kits, menu's, injury reports, medications and medication authorizations were reviewed. Areas of non-compliance were cited on the violation notice. The children were observed playing with floor toys, making bean bags and playing games with the staff. Based on a review of children's records, the center did not obtain documentation of a physical for each child enrolled. by the 30th day of attendance

Evidence:
Child #3 and child #4's records do not contain documentation of a physical examination by the 30th day of attendance, Child #3 and #4's first day of attendance 8/23/21.
Plan of Correction: I will contact parents and request a copy of the physical.

Based on observation the center did not ensure that a 911 or local dial numbers for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone.

Evidence:
The center did not have 911 or local dial numbers for police, fire, emergency medical services or poison control posted in a visible place at each phone.

Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
Child #3's record did not contain documentation of the first day of attendance.
