Inspector Notes
An unannounced monitoring inspection was initiated on 7/10/2025 and completed on 7/30/2025 in response to a self-report received by the licensing office on 7/9/2025 relating to behavioral guidance. This inspection include document review, interviews and a review of video footage.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to (area or part of standard) and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/14/2025. A POC submitted after this date will not appear on the public website.
Violations
2The programs Operations Manual states "Personal electronic devices may not be used during SACC time." Two video clips from child #2s smart watch shows staff #1 using their personal cell phone while supervising a group of children.
1. A video recording from child #2s smart watch shows staff #1 discipling a group of children who had finished eating lunch. The video recording shows staff #1 saying "You don't want to be here? It's so annoying. You can't just sit here and behave yourself until lunch is over?"
2. A video recording from child #2s smart watch shows staff #1 disciplining a group of children that was asking staff #1 why they are on their phone. The video recording shows staff #1 saying "You don't talk to me like that. That's none of your business. You don't talk to me like that."
Staff #1s tone of voice when speaking to the children was negative, dismissive, confrontational. Staff #1s guidance was not intended to resolve conflict of redirect children's behavior.
