Inspector Notes
An unannounced monitoring visit was conducted today. Two daycare children and two of the provider's children were present with one caregiver for seven points which is within ratio for supervision. Children were observed finishing a snack, free play and watching a show on tv. Home used the living room, bathroom and back bedroom for sleeping (cribs are in the back bedroom). Reviewed physical space, records, training certificates, household member health records and emergency preparation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Listerine mouthwash and cleaners were on the back of the toilet and not out of reach of children.
Evidence:
The 2 outlets behind the crib were uncovered.
Evidence:
Child #1 and Child #2 were missing health insurance policy number.
Evidence:
Soiled diapers were visible in the open trashcan in the bathroom.

Child #1 was missing the signed acknowledgement of the "Information For Parent" form that provides the parent information on the Family Day Home policies and procedures.

The provider stated that bug spray and diaper ointment are used but no forms have been completed by the parent.

Evacuation drills had not been documented from April 2022-July 2022.
