Inspector Notes

An unannounced renewal study was conducted today. Four children were present (two daycare and two of the provider's own children0 for six points which is within ratios for supervision. Observed the children playing with blocks and books, hand washing and transition to lunch. Lunch served today was hamburger meat and rice, cantaloupe and veggies. Reviewed physical space, records, trainings, emergency preparation and nutrition. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Evidence:
Child #1's last immunization was dated 1/2022.
Plan of Correction: Provider will request the parent update the shot record to reflect the most recent immunizations.
