Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on 1/26/2023. There were 4 children present with 1 staff supervising. Two of the 4 present were the provider's own children. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on re3cord review and vendor statement, the subsidy annual update was taken within a year of the preservice training.
Evidence:
The preservice was taken in December 2021 and the Health and Safety update should have been taken by December 2022. At the time of the inspection it still needs to be completed.
Plan of Correction: Vendor will complete the training in the next 2 weeks.
