During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/22/2024, as a part of the licensure period. The on-site inspection began at 11:30am and ended at 12:25pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 1 caregiver; the point total was 10. The inspector reviewed 4 children?s records and 1 caregiver/household member(s) record on site.

This inspection included: document review (i.e. Injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms), tour of the facility, interviews, observations (this would include observations of photos, recordings, etc.), and measurements

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
