This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an onsite tour of the program.
An announced non-mandated inspection was initiated on November 29, 2021 and concluded on November 29, 2021. The provider requested a license modification to change the capacity based on the use of additional space. One additional room was observed and measurements were taken.
