Inspector Notes

An announced initial inspection was conducted on 9/23/21 from 9:45am to 12:10pm. There were no children present during this inspection. A walk through of the facility and playground was conducted. All space intended to be used by children, including the playground was measured and assessed. 3 staff records were reviewed. The emergency preparedness plan, supplies and first aid kit was reviewed. The classrooms had table top toys, puzzles, , books and other curriculum related manipulatives available. There were no medications on site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22VAC40-185-270-A
Based on observation, areas inside and outside the child day center were not maintained in a safe and operable condition.
Evidence:
1. The air conditioning units by the windows in Rooms # 2, 3, 4 and 5 were observed to be rusty.
2. A rectangular panel on the outdoor play structure by the slide was observed to be rusty.
3. Ceiling tiles in the following rooms showed signs of water damage: 8 ceiling tiles in the hallway; 4 ceiling tiles in Room # 2; 3 ceiling tiles in Room # 4; and 1 ceiling tile on Room # 5.
Plan of Correction: Playground panel has been sanded and painted. Air conditioning units will be painted in all rooms. Ceiling tiles are being replaced through out the areas listed in violation.
