Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A non mandated monitoring inspection was initiated and concluded on 1/13/22. This inspection was conducted to assess the care of children during the conditional period of the center. The director was contacted by telephone and a virtual inspection was conducted. There were 6 children in director care and supervision of 3 staff members. The children were observed engaged in story time. Previous violations related to the physical space were corrected. A total of 2 children's records, 3 staff records and other center documents were reviewed. There were no medications on site.

Standard 22.1-289.035-A
Based on records review and staff interview, the center did not obtain results of the VA central registry findings for an employee within 30 days of employment.
Evidence:
1. Staff # 1's date of hire was 11/15/21.
2. Staff # 2 stated that the VA central registry form for Staff # 1 was mailed but the results had not been received.
3. Staff # 2 stated that a follow up was not completed to check on the status of the results.
Plan of Correction: A follow up email was sent to the agency and the staff member has been asked to complete a new form to mail out again. A follow up will be completed with the licensing inspector when the results are obtained.
