Inspector Notes
An unannounced, on-site monitoring inspection was conducted on 9/16/25 from 8:15 am to 9:30 am. The inspector reviewed compliance in the areas listed above. There were 13 children present and 6 staff. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Staff # 1 has been employed at the center for 3 months. An out of state CRC was not submitted prior to the first day of employment, or to this date.
