Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on-site April 17, 2023. There were 5 children present with 1 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Based on review of the attendance record and interview, the vendor failed to ensure to maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence: The attendance record did not contain documentation of the arrival of each child as it occurred. Caregiver #1 confirmed the attendance record was not updated.

Standard 8VAC20-790-250-B
Based on a review of caregiver records and interview on April 17, 2023, the vendor failed to ensure within the first 90 days of subsidy vendor approval, all caregivers complete the Virginia pre-service training for child care staff sponsored by the Department of Education.
Evidence: The record of caregiver #2 did not contain documentation of the Virginia pre-service training. The provider was approved for subsidy on December 13, 2022. Caregiver #1 acknowledged the training was not completed.
