Inspector Notes

Today an announced Renewal Inspection took place between the hours of 11:30am and 1:00pm. There were 5 classrooms observed with 44 children supervised by 9 staff. Children were observed having "recess" in the mosque, writing in work books, participating in circle time and other organized activities. A complete inspection of the physical plant and staff records were observed during this inspection. Violations were found during todays inspection and can be viewed on the Violation Notice.

Based on record review and interview, each person has not been certified by a practicing physician to be free from any disability which would prevent them from caring for children under their supervision.
Evidence: Staff #8, Staff #12 and Staff #13 have not submitted documentation of a staff health report. Center management confirmed that all staff have not provided a health assessment.
