Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on September 14, 2021 and concluded on September 15, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 28 children present, ranging in ages from 16 months to 5, with 6 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and staff interviews, the licensee did not ensure a menu listing foods to be served for meals and snacks during the current one-week period shall be posted in a location conspicuous to parents or given to parents.

Evidence: The menu posted in the Toddler 1 and Toddler 2 room did not include the snacks to be served for the current week. The menu posted only listed the snack for Monday, September 13, 2021.
Plan of Correction: Instead of posting the snack daily, the snack menu will be set for the entire current week. At the time the report was received the snack menu had been updated.

Based on record reviews and staff interviews, the license did not ensure the child day center had obtained a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years. The criminal history record information check and a sex offender registry check must be obtained before reporting to the child day center.

Evidence: The record for staff #1, first date of employment 8-24-2021 did not include the sex offender registry conducted in the state of Florida where Staff #1 had resided within the last 5 years. Staff #1 was working during the inspection on 9-14-2021.
Plan of Correction: The sex offender registry will be completed and the employee will be removed from the schedule until results are obtained. At the time the report was received, the check had been completed and submitted.
