Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on February 3, 2022 and concluded on February 4, 2022. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 18 children present, ranging in ages from 16 months to age 6, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interviews, the licensee did not ensure the child day center had obtained a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years, before reporting to the child day center.

Evidence: The record for Staff #2 did not include a criminal records check prior to working. Staff #2 date of hire 1-31-2022 and was observed working during the inspection.
Plan of Correction: Although Federal and state clearance had been obtained, the Kentucky background check had not been sent. It was sent out on Feb 4, 2022.

Based on record review and staff interview, the licensee did not ensure to obtain from employees documentation of the tuberculosis screening at the time of employment and prior to coming into contact with children.

Evidence: The record for staff #2 did not include docuemtniaon of a TB screening prior to working with children. Staff #2 date of hire 1-31-2022 and was observed working during the inspection.
Plan of Correction: Tb test will be performed immediately and June test was in file.
