Inspector Notes

An unannounced monitoring inspection was conducted from 2:25 PM -4:20 PM. At the time of entrance, 25 children were in care with 5 staff members present. The sample size consisted of 5 children?s records and 5 staff records. Children were observed during rest period, snack time and playing in small groups. Areas of non-compliance are identified on the violation notice.

Based on record review, observation and staff interviews, the licensee did not ensure that there shall be in each grouping of children at least one staff member who meets the qualifications of a program leader or program director.

Evidence: Staff #5 was the only staff supervising 7 children (ages 3-6) in room #110 during the inspection. Staff #5 verified the Lead teacher leaves at 1:00 and they are the only staff supervising the children. Center Director confirmed Staff #5 does not have the documentation that indicates they meet the qualifications of a program lead or program director
Plan of Correction: After reviewing staff #5 training records, it was found that records were missing from the file. Training records were updated to reflect that staff #5 had appropriate qualifications. All staff files will be reviewed to ensure that qualifications and training are properly documented.

Based on record review and staff interviews, the licensee did not ensure staff records included all required documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering and included all required documentation.

Evidence: The records for Staff #1, #2 and Staff #5 had inadequate reference check information they did not include a signature from the person who conduct the reference checks.
